Debriefing
Incidents happen. It is an unavoidable reality. However, if organisations do not learn from these events, the likelihood of them happening again increases significantly.
Unexpected incidents can have a major impact on operations, safety and organisational confidence. What truly matters is the ability to learn from these situations so that, if something similar were to occur in the future, the impact is reduced and improvements have been implemented. A structured and meaningful debrief is essential to achieving this.
But debriefing is not only valuable after major incidents. Routine debriefs, held after any event or activity even when everything appears to have gone well, can be just as beneficial. These discussions often highlight small issues that may have gone unnoticed by some but clearly observed by others. They can also capture examples of good practice, successful actions and things that should be repeated in the future. Regular debriefing strengthens organisational learning, builds trust and encourages continuous improvement.

We work closely with you, and with partners where required, to develop clear and relevant terms of reference for the debrief. This ensures that the process delivers exactly what you need, with a strong focus on identifying valuable lessons that will improve future performance and preparedness.
You will be supported by an accredited and experienced facilitator throughout the planning and delivery of the debrief. Confidentiality is maintained at all stages. We can conduct debriefs in person at a location of your choice, or remotely using video conferencing platforms such as Microsoft Teams or Zoom.
Following the debrief, we will provide you with a comprehensive report outlining key findings and a set of practical recommendations. These can be used to prevent similar incidents in the future, strengthen organisational learning and support continuous improvement. If required, we can also help you track, monitor or implement these recommendations.
